There are a lot of drawbacks with free AV software from display ads and pop-ups to limited features and reduced performance, so why would anyone choose to put up with this? The main drawback to upgrading is of course, cost. Some people don’t spend enough time on their PC to justify spending money on software that they can get for free.
#1: Sort all files by size and delete old large files. Open Finder, go to Recents, and look at the Size column. Click on it to sort all recent files by file size (from large to small). You'll have a clear overview of what items are eating up a large amount of space, e.g. From 1 GB to 10 GB, and from 100 MB to 1 GB.

If you tend to use your computer to play offline games like solitaire, to create documents and have limited internet access then a free anti-virus will probably suffice. On the other hand, if you are a heavy online user that accesses internet banking and social media sites everyday, a free version will not cut it.
Mac's don't need defragging as OS X automatically writes 2Gb or below sized files to one contiguous space on the drive. There are certain cases where's it's needed to free up a large spaces on the drive, but not something everyone has to do.
Far as other Mac maintainence, there really isn't any except to backup one's files regularly. Keeping Software Updated and run a web browser check to make sure the plug-ins are current.

If your friend got onto your computer and visited a bunch of adult sites, then I would recommend something like OnyX, doing ALL the cleaning and maintenance aspects and rebooting, perhaps followed by a Disk Utility Erase free space.
CCleaner beta is out for OS X, used on PC's and combines cleaning and scrubbing of the drive if setup correctly.
